| dc.description.abstract | การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อศึกษาสภาพปัจจุบัน ปัญหาและอุปสรรคและพัฒนาวิธีในการจัดหาวัสดุครุภัณฑ์ทางการแพทย์ของโรงพยาบาลชุมชนเขตบริการสุขภาพที่ 2 (พิษณุโลก) ใช้รูปแบบการวิจัยเชิงคุณภาพ เก็บรวบรวมข้อมูลด้วยการสัมภาษณ์เชิงลึก และสนทนากลุ่มย่อย ผู้ให้ข้อมูลสำคัญ จำนวน 72 คน ได้แก่ ผู้อำนวยการโรงพยาบาล/ นายแพทย์สาธารณสุข หัวหน้าฝ่ายบริหาร หัวหน้างานพัสดุ หัวหน้ากลุ่มการพยาบาล คณะกรรมการกำหนดสเปค และคณะกรรมการตรวจรับ ผลการศึกษาพบว่า วิธีตกลงราคา สภาพปัจจุบันการจัดหาวัสดุครุภัณฑ์ทางการแพทย์ของโรงพยาบาลชุมชนบริการสุขภาพที่ 2 (พิษณุโลก) พบว่า การดำเนินงานตามระเบียบสำนักนายกรัฐมนตรีว่าด้วยการพัสดุ พ.ศ. 2535 ปัญหาอุปสรรค พบว่า กฎระเบียบในการปฏิบัติไม่ชัดเจนมีขั้นตอนหลายขั้นตอน ในอนุมัติเกิดความล่าช้ามีระยะเวลาจำกัดในกระบวนการจัดซื้อจัดจ้างการพัฒนา วิธีตกลงราคา ประกอบด้วย ผู้เสนอราคาแหล่งข้อมูลไม่น้อยกว่า 3 ราย ไม่พิจารณาราคาถูกเพียงอย่างเดียว วัสดุครุภัณฑ์ตรงกับความใช้งานผู้ใช้งาน รายงาน e-GP ให้เป็นปัจจุบันยกเลิกบันทึกเอกสาร เบิกจ่ายตรงจากสำนักงบประมาณ และยกเลิกเงินสนับสนุนสวัสดิการ วิธีสอบราคา สภาพปัจจุบัน พบว่า วงเงินในการจัดซื้อวัสดุครุภัณฑ์เกิน 100,000 บาท แต่ไม่เกิน 1,000,000 บาท ตามระเบียบพัสดุ พ.ศ. 2535 ปัญหาอุปสรรคพบว่า ระเบียบปฏิบัติไม่ชัดเจน มีระยะเวลาจำกัดในกระบวนการจัดซื้อ อีกทั้งปัญหาด้านเทคโนโลยีสารสนเทศระบบที่ล่าช้าไม่เสถียรและกรอบกำหนดระยะเวลาในการบันทึกข้อมูล การอนุมัติงบประมาณช้ากว่าแผนงบประมาณประจำปี การพัฒนาวิธีสอบราคา ประกอบด้วย เปิดกว้างวิจารณ์ร่าง TOR (แบบลับ)/ ผู้ขายร้องเรียนแบบลับ คณะกรรมการกำหนดสเปค (อิสระ เปิดเผย) เบิกจ่ายตรงจากสำนักงบประมาณ มาตรการลงโทษที่รุนแรงเมื่อมีการทุจริต วิธีตลาดอิเล็กทรอนิกส์ หรือ e-Market สภาพปัจจุบันพบว่า โรงพยาบาลชุมชน ส่วนใหญ่ไม่ได้ใช้วิธีวิธีนี้ การพัฒนาวิธีประกวดราคาอิเล็กทรอสิกส์ (e-bidding) ประกอบด้วย คณะกรรมการตัวแทน (อิสระ) เป็นกลางกำหนดสเปค ผู้ไม่มีผลประโยชน์ร่วมวิจารณ์ร่าง TOR (แบบลับ) กำหนดหลักประกันการยื่นซองเกิน 1,000,000 บาท/ โครงการ วิธีประกวดราคาอิเล็กทรอนิกส์ หรือ e-Bidding สภาพปัจจุบัน พบว่า กระบวนการการได้มาของงบประมาณถูกเสนอจากระดับล่างขึ้นบน ในกระบวนการ e-GP ที่มีขั้นตอนมากเชื่อว่าเป็นกลางไม่เอื้อต่อบริษัทใดบริษัทหนึ่ง ในเรื่องระยะเวลามีความคุ้มค่า ครุภัณฑ์ที่ได้ไม่สอดคล้องกับงบปรเมาณมีความเป็นกลางเปิดกว้างสามารถวิจารณ์ร่างสเปค (TOR) ปัญหาอุปสรรค พบว่า ระบบไม่สามารถป้องกันในความโปร่งใสและตรวจสอบ, ระบบอินเทอร์เน็ตช้า โปรแกรมไม่รองรับขาดอุปกรณ์สนับสนุน การพัฒนาวิธีประกวดราคาอิเล็กทรอนิกส์ หรือ e-Bidding ประกอบด้วย สถานที่เก็บประเมินอัตราการใช้ใน 1 ไตรมาส เปิดกว้างให้ผู้ค้ารายย่อย (ท้องถิ่น) เบิกจ่ายตรงจากสำนักงบประมา | |

| dc.description.abstractalternative | This research was aimed at to investigate the current conditions, problems and barriers and the procurement methods development for medical supplies of the community hospitals public health region 2 (Pitsanulok). A qualitative approach was applied for data collection through conducting in-depth interview and focus group with 72 key informants, who were the Hospital Directors/ the Public Health doctors, Chiefs of Administration, chief of Supplies, Chiefs of the Nurse Group, the Specification Committee and Supplies Check Committee. Results revealed that: Price Agreement Method-the current medical supplies procurement of the community hospitals public health region 2 (Pitsanulok) disclosed that the procedure was adherent to the regulations of the Office of the Prime Minister on Supplies BE 2535 (1992). Problems and barriers were rules and practices were ambiguous with red tape and approval delay with restricted time in the procurement procedures. The Price Agreement Method Development contained not less than three bidders, not considering just cheap price alone, users-fit supplies, updated e-GP reports, cancellation of paperwork but direct disbursement with the Budget Bureau and cancellation of welfare funds. Price Checking Method currently showed that the financial margin for supplies procurement greater than 100,000 Baht but not exceeding 1,000,000 Baht under the Supplies Regulation BE 2535 (1992) met the problems and barriers that the regulatory practices were ambiguous with restricted time for the procurement. Also, the information technology was slow and instable as well as the time frame for recording and the approvals were slower than the annual budgetary plans. The Price Checking Method Development contained being opened for critiques on TOR (confidential)/suppliers confidentially complained, the Specification Committee (independent and transparent), direct disbursement with the Budget Bureau, and rigid punishment measures upon corruption found. The e-Market Method in the current conditions showed that most community hospitals decline to use it. The e-Bidding Method Development comprised the representative committee (NGO) was impartial to set specification, non-stakeholders cocommenting on TOR Draft (confidential), and setting amount of bidding guarantee for more than 1,000,000 Baht/ project. The e-Bidding Method currently showed that the process of budget grants was proposed from bottom. The e-GP process with multiple procedures was believed that impartiality was found and without facilitation to any companies. Time was worthwhile. Supplies irrelevant to budget are impartially opened for criticism on TOR Draft. Problems and barriers revealed that the system disabled to protect transparency and audit ability, slow internet, and unsupportive programs needed supporting devices. The e-Bidding Method Development comprised a space to store using rate estimation for a quarter, being opened to local suppliers and direct disbursement with the Budget Bureau. | |
